HEMA Alliance Insurance Waivers

Postby John Harmston » Thu Feb 17, 2011 2:50 pm

Just a quick housekeeping item. Remember that if you belong to an insured affiliate club, there should be a signed waiver in the possession of your club/school for every insured participant.

The waivers (there are two that need to be signed each person) can be found at: http://hemaalliance.com/?page_id=1019

If you are unsure of whether your club is insured or is simply a non-insured affiliate, check with your club representative or a member of the alliance's Governing Council.

Thanks.

Didn't know about the second one. Thanks for the reminder!

Does the "received by" need to be filled out or signed by anyone in particular (or at all)?

Lemme see...

Ok, waivers should have a signature from those signing it, and those accepting it. So, in Tyler's case, those attending his event will sign the waiver, Tyler, or an appropriate minion will sign the received by. This prevents a yahoo showing up to the event with a waiver he signed, but no one else acknowledged.

Guys, just a quick request here, PLEASE try not to change the links to the website around too much, especially without letting Affiliates know you've done it. I get the desire to do some housecleaning, but just like the link at the top of this very thread is no long valid, so too were all of the links I had on our website to where the waivers, safety policy and insurance information was at, all things that are VITAL for people interested in our club to have ready access to. The less those links change the better it will be for everyone in reducing a LOT of confusion I'd been working with lately (until I came here to figure out why folks were telling me they couldn't get these vital documents, and now I know it's because all the links changed!)

We rebuilt our website.

http://www.hemaalliance.com/documents/

Or, go to the hema alliance page, go to the bottom where it says documents, and have at it.

We will do housekeeping and change things from time to time, but our website will have vital documents on it.
